Help with a Chevy S10, tranny or 4 wheel drive??

So I let my Grandmother use my truck for 4 days while her car was being repaired... When she gives it back to me I notice a odd sound from under the truck when I shift to 2ed and 3ed, not 1st, or 4th. I'd say it sounds like a grinding.. metal on metal..? I just got the truck and as I look over the inside I see the 4 wheel drive isn't in the 2 wheel drive or 4 wheel drive even its inbetween 4 wheel high and neutral.... did this cause this? she drove about 45-50 miles like that at least before I set it right.... If not what would you say it is... Thanks people I'm just a student, and am not too car savy..

What year is this? You're saying the transfer case shift lever is down between 4h and N? Is your 4x4 engaged? Does this imrove if you move it back to 2H? If you double clutch the upshifts does it make this noise? Is the noise present on your downshifts? What time did granny get in the 1/4 mile while she was using the truck?
