2007 Impala transmission problems

I've got a 2007 Chevy Impala w/ 3.9l engine auto trans. It has 78,000 miles on it. The transmission act like it goes into neutral when you go to take off then it slams back into gear and jerks real hard. It does it mostly at take off but sometimes you might be going about 15 when it does it. It only does it when accelerating. It seemed to do it worse this weekend after a trip of about 200 miles. I changed the fluid and filter at 75000. Didn't see anything unusual in the pan and the fluid wasn't that bad. It was doing this before that and seems to be getting worse. My wife drives it daily so I'm not sure how often it actually does it.

Check to see if there are any trouble codes being stored in the engine computer. Write down the numbers.
