2005 Accord Clutch Issues?

I have a 2005 Honda Accord, 4 cyl, manual 5 speed transmission. When I attempt to rapidly accelerate, the RPMs rise rapidly and then the power is transferred to the wheels (like I'm riding the clutch, but I'm not). Do I need a new clutch or just an adjustment?

That's a strong sign of a worn out clutch. Not adjustable to make it grab harder,
