1994 Chrysler Town & Country mini van transmission issues, need help

I have a 1994 Chrysler Town & Country mini van with 146k on it. Everything has been fine until recently.
Driving at 60 MPH, the RPMs began running high and its stuck on 3rd or 4th gear. I can’t drive any faster or it’ll redline. Also reving at low gears.
What do you think the issue is & how much to repair it. Looking more and more like the tranny has finally failed.

I'd have the transmission control module (TCM) scanned to see if there are any trouble codes being stored. Need enhanced scan tool that can access the TCM to retrieve codes. The TCM could be defaulting the transmission because it detected a problem.
Since we volunteer our time and knowledge, we ask for you to please follow up when a problem is resolved.
