My car jerks forward violently after driving about 15 min

After about driving 15-20 min. my car starts to lose acceleration out of no where (this happens regardless of speed). I have to press on the gas for about 5 seconds or so and it will start to accelerate again, but it will accelerate very hard and dart forward. If I just coast it will still jerk forward but not as hard as when I press on the gas. However if I just put it in neutral then it coasts fine.
Before this happened I cleaned my EGR valve, and had to remove a bunch of hoses. I looked at them and it LOOKS like they are all on snug. I also cleaned out my intake manifold. After I did this, that is when the acceleration problems happened.
