odd engine trouble

I have a 1996 gmc K2500 6.5l diesel.
When the engine is well warmed up, it will begin to almost try to rev up on it's own. It's very short and quick little revs.
Then when I'm driving at road speed it will be working fine, but suddenly it will get really loud(the engine). It does not seem to loose power, but it gets really loud and sometimes smokes black really bad.
?pump?EGR?
I'm sort of at a loss as I don't have a lot of diesel expierience.
Truck also has 400+ kilometers on it. Pump was done at about 250.

MAS; Not a diesel mechanic, here, but sounds like an injector pump problem.
