Oil Cooler leaking coolant

I have a 2001 Chrysler T&C w/3.8L engine. I have an coolant leak coming from the oil cooler. I am looking for a part number for the oil cooler. The oil filter spins on to the oil cooler.

Unless aftermarket just ask for the part you want. If aftermarket you'll need to know what brand it is,
