Distributor and ICM on 1994 E350 460 7.5L Sign In

Our church has a 21 passenger shuttle bus I'm working on. It has been faltering/surging (while in park, neutral, or drive). I replaced the cap, rotor, plugs, wires, air & fuel filter, but the problem was still there. It also failed emissions testing recently. We took it in for a diagnostic, and they said it was running rich and lean (don't know how that is possible, but ok). They said the daig showed cylinders dropping out. They recommended replacing the Ignition control module and the cylinder pickup assembly. They wanted $750 to do it, so I figured I'd do it myself.

I took off the cap, rotor, and as much of the top of the distributor as I could, but I couldn't get the cylinder pickup assembly out. I figured maybe I have to remove the distributor and pull the gear off the bottom and pull the shaft out through the top. I removed the two keepers at the bottom of the shaft, but the gear seems to be really on there. One thing I noticed was that the shaft has some play in it, moving up and down a good 1/8th inch. So I got to thinking, maybe that is the problem, and the pickup assembly is good, but the distributor is bad.

Oh, and I also noticed the ICM was missing. there was the area it clearly should be on (the side of the distributor), but it was not there. there wasn't even a wiring harness anywhere near the area. Could it have been moved in an after market sort of way? Maybe it doesn't have one?

So, in summary:

1. Should I replace the distributor, or just the cylinder pickup assembly?
2. If pickup assembly, how so? do I need to use a gear puller, or should the top end of the dist. come apart more?
3. What's up with the ICM?
