I've had it a year. It had 181K on it last spring, so it hasn't known such heavy use in 30 years. However, it hasn't had a single unfixable (or at least inexplicable) problem until this week.

At the very end of the same 15 mile commute it makes 4 days a week, it spontaneously revved itself to full tilt, blowing an enormous cloud of smoke out the tailpipe. It ran itself dead by the time I could stop and run to the other side of the road.

- the accelerator was not stuck
- the engine stopped driving the wheels as soon as it started doing this
- the engine continued running at full power when the ignition was turned off and the key removed
- the engine stopped only when oil was gone
- other than a wiggle stemming from aging electronics in the instrument cluster, the oil gauge gave no warning
- there were no flames and minimal smoke in the cabin and under the hood

So, I'm wondering a) what the hell happened? and b) what damage did it likely cause the engine?

I don't do a whole lot of diesel work but it sounds like oil was getting into the cyls and the engine was running on the oil. Diesels are a compression ign system (no spark plugs) Once they get to operating temp an injector injects fuel into the cyl and the compression ignites it. If another (unregulated) type of fuel is able to enter the cylinders then the engine will run on that until the fuel is gone.

Basically your engine oil is somehow getting into the cyls & the engine is running on it...... It's going to have to be checked out to see how the oil is getting in there & fixed. The problem will have to be fixed & then see if there is any damage. If it didn't start knocking or making any weird noises before it shut down, you may be ok......

This is called a "runaway engine." As Sidom said, it burned its oil as fuel. If this is a turbocharged engine, a turbo failure is the most likely way to get this much oil down the intake. Diesel engines regulate engine speed by the amount of fuel they allow, not air. Diesels don't need the 14.7:1 stoich ratio of gasoline, running anywhere from 13:1-100:1 depending on load and boost levels. Because of this, they will burn oil as readily as fuel and when enough is dumped in the intake it is unregulated amounts of fuel. The result is what happened to you. You need to take off the oil pan and check the rod and main bearings to see if they suffered damage. Unfortunately, they likely have. If they haven't go get a lottery ticket as its your lucky day. Look at the impeller of your turbo and the seal for it. Realize if its the turbo that went this is more than just bolting it on. The intake will need to be taken off and cleaned out, along with all air ducting, to remove oil and metal fragments of the blown out. Each individual piece of ducting must be separated to ensure the particles are out. If this has a charged air cooler it will also need to be removed and cleaned very thoroughly. I'm not certain about your car, as I'm not a Euro guy, just used to work on semi trucks and tugboat diesels. The old 2-cycle Detroits were good for running away. Saw one go up in a tug that we evacuated the engine room and stood by with a fire team until she went!
