94 chevy 2.2 exhaust valve push rod plates breaking!

Hello , I have a 94 Chevy Caveleir 2.2 standard. I recently blew the head gasket and the head cracked. It ran a bit hot and was blowing water out of the exhaust. We replaced the head with a brand new head. It ran fine for about 10 miles and some of the exhaust valve push rod guide plates broke causing the rocker arms to come off of the valves. We took the new head back to the shop and got another new head this time replacing the lifters and push rods. We drove it 10 miles and the same thing happened again. We are stumped! Has anyone ever ran into this problem before? Any advice would be greatly appreciated!

The only thing that I can think of is the installation procedure of the rocker arms is somehow wrong, like over-torqueing maybe? Are you sure the camshaft didn't warped?
