1998 chevy malibu 170,000 miles serpentine belt fell off due to crank shaft pulley out of alignment what caused this.

Crankshaft pulley AKA "Harmonic Balancer" is both a balancer and pulley drive source and can come apart as it's both rubber and metal.

They are available and it's a must do now job. Belt won't behave with that out of place,

To add; Many times, the deterioration of the rubber in between the two pieces is caused from an external oil leak. And, the pulley/harmonic balancer does just that....balances the crankshaft. If left go, will 'shake' the engine apart.
