1997 Ford Escort 100K miles - engine lose power when hot, engine 'gaps' on acceleration

My Escort (manual transmission) runs fine unless I sit in traffic. Then for some time engine is not working smooth when accelerating - 'gaps' in the pattern. Comes back to normal after few miles on highway. What could it be? Changed air and fuel filters, replaced few old-looking vacuum lines on engine - no effect.
