many things it could be. vacuum leaks IAC (idle air control valve) Air restriction (gummed up thorrotle body, plugged filter) EGR stuck partially open ... could be more things Is check engine light on, if so what codes are you getting?.

I had the same problem, with no codes. I pulled the fuel line off and it worked fine, but I did not check fuel pressure. When I unplugged the temp sensor on the front of the engine, it ran at a crazy high idle. Well, it turns out it was a 2 inch hose, inside the fuel tank was split. It went from the fuel pump to the housing. When the pump began to build up pressure, the line opened up and shot fuel BACK into the tank. That caused the throttle body to starve for fuel, thus running like crap, then running great, then running like crap again. It was a 1 dollar fix, but i had to drop the tank to access the pump.
