v8 fault code P1345. Can anybody give me some insight on this one

Yep, the distributor is not installed correctly. I suspect someone thought they could change the timing and turned the distributor in an attempt to do that. The timing is not adjustable. The distributor has to be adjusted correctly using a professional scan tool and adjusting the cam offset to 0.

It's just an adjustment. It shouldn't cost more than 1 hour labor.

my other question is what kind of problems would that contribute to in the truck?

It will cause all kinds of driveability issues. You have thrown the injectors and the secondary ignition totally out of synch with the primary trigger. It needs to be resolved before it does Catalytic Converter damage.
