Engine rattling while running (worse at low speed/idle)

My 99 Honda Odyssey is making a rattling sound when I start it up and at lower speeds. Once I get it up over about 40 or so, it goes away. It's a rhythmic rattle. I think it may be belt related. Any suggestions?

If you take the belt off you can get a better idea of how the pulleys and their bearings are. Also check for exhaust heat shields that get rusted and rattle. Common on some vehicles, T
