99 saturn sl auto trans hesitates going into reverse

my 99 saturn sl hesitates when going into reverse when warm. also, it at times would not shift from 3rd into 4th. These issues do not occur when the car is cold. it does make a clunking noise. to get it into reverse when warm, I have to apply the brakes and press the accelerator. It can jerk violently if I give to much input to the gas pedal.
