The overwhelming reason is the cooling system has vapor in it or air passing thru the core. You could feel that with one hose hot and the outlet hose stone cold with it operating full blast on heat.

Why? Because it was plugged strongly suggests lack of antifreeze service and possible bad luck it clogged and many are part of the by-pass of the cooling system to keep engines a uniform temp. Why? So you don't get hot spots in an engine and blow head gaskets which is what I would be looking for with this complaint. If found that is the culprit need to totally check out corrosion issues for the whole system and correct if possible,

The main reason a heater core gets plugged up is because the cooling system is sludged up with rusty sediment. Clearing one component in that system does nothing for the rest of the system and it plugs right back up again.

For the moment I would disconnect the the heater core and backflush it thoroughly with a garden hose. Leave it bypassed while you find a way to clean out the rest of the system.

That should include temporarily removing the t-stat and circulating a very strong chemical to loosen up all the sludge. I have seen people resort to using muriatic acid but you have to be real careful with that stuff.

Also, don't be surprised if cleaning out the impacted sludge exposes leaks in the system that will have to be repairs. There is likely rot sitting under a lot of that sludge that will now be exposed.
