1990 honda accord temperature gauge show that vehicle is hot and then the cooling fan comes on and the gauge back down a little ways from hot but the motor is not that hot. So which is at fault the temperature gauge, coolant temperature gauge sending unit ,or coolant temperature sensor for the ECM ?

I can't say what the car's gauge thinks is "hot" vs a real temp. In that, that gauge indicates temp coming down when fan(s) kick on I'd say it is running hot so get a temp reading of what it really is. Ifra red touchless thermometer is easiest or a probe (meat thermo) right at thermostat will tell you what the real temps is. Coolant could read "hot" at 240F or something and not be boiling over if pressure cap is working and that's too hot. When an engine goes over 260F boil right over and take out any doubt.

Notes: Boiling points.... at sea level....
Plain water - no pressure 212F boils
Plain water with 15lbs pressure - 257F boils
50/50 with anti-freeze raises #s a bit but not much,
