truck hesitates, coughs and sputters...looses power

my '96 chev half ton runs and revs fine in neutral but in gear it hesitates, coughs and sputters with lack of power...acts like it wants to go but something is holding it back...any suggestions...

scan for codes- even if the CEL is off.
Do a exhaust backpressure test - a engine vacuum reading can be used.
Always with fuel injection- fuel pressure test-
Tell us any modifications recent work etc.
