Sparkplug from # 4 Cylinder is carbonized and truck requires a little more gas to smooth out the roughness at low speed. Highway traveling seems to be pretty smooth but in city driving it appears to be a little rough.

Question: Does this seem like it may be a bad valve or bad pistion ring (are these the same thing )that can only be corrected by having the engine pulled ?

Is there any other way to correct this or minimize this beside replacing the spark plug in # 4 cylinder often ?

Will I be damaging the car anymore by driving it without correcting the problem ?

Sounds like you may have carbon deposits built up in the cylinders. Have a shop perform a top engine clean to remove the carbon. You will have to call around to find a shop that has the setup to do this.
