96 Chevy Blazer Rough Ride!!

I need some serious help. 96 Chevy Blazer, 200k very rough idle. Shakes real bad and feels like its about to turn off. Also this loud hissing noise is coming from somewhere around the distributor. Already replaced distributor n cap n rotor.. Plugs n cables, Gas filter. Can it be some valve or sensor that needs replacing? Suggestions por favor.

A hissing noise coupled with running bad is almost always a vacuum leak. Look around the area at hoses and connections to the intake and find the culprit. Should smooth out instantly when leak is plugged up.
