1998 Nissan Sentra automatic makes rattling sound in all gears but neutral

At idle and while driving, the car makes a rattling sound like loose bearings. The sound is there in all gears but neutral. Car has 285000 miles. The sound has been there as long as I have had it (2 years) and is maybe getting a little worse. No obvious problem with the transmission or engine. Any ideas?

285,000 miles is enough for anything in the vehicle possible! No matter but look for simple like something exhaust - heat shields, hangers, clamps or touching where it shouldn't. Can check by tapping on exhaust with engine cool even with a rubber hammer and usually find it if that's it,
