I started hearing scratching noises 3 days ago and immediately ordered the brake pads. I never heard any high pitch squealing.
The back passenger brakes had about 60% left... and then there were the back drivers. Pictured above.
I purchased this vehicle as a Toyota Certified 15k miles ago. I have no idea why the one side is so much worse than the other. Maybe he dealer replaced the right side and left the other alone? The sets of pads do not have any distinctive markings for me to tell if they are different brands.
I replaced both, but my question is this
Do I need to replace this rotor?
In my mind, there is about 3/8" of scratched rotor on the outside. This shouldn't be so detrimental to the life of the pad since I still have the 1.25"+ left on the rest of the smooth rotor to work with. Please let me know if I'm totally off base.

No, you can't use that rotor. It would have to be measured to determine if it is thick enough to resurface but I doubt it. You should replace both rotors.
You also need to determine the cause of the uneven wear. It could be frozen slides, bad caliper or brake hoses.

any suggestions about how I can figure out just what is wrong?
It was fairly hard to pull the brakes off, so maybe the slides were seized up (i'm just hoping because that seems by far the cheapest thing)
I did not lube the new slides, should I do that every time I change pads? These rear pads did not come with lube, the front ones did but I didn't have to change those, so I saved the lube.

These are all the reasons that brake diagnosis is not a DIY job. You need a professional to recognize the problems with your brake system. Brakes aren't OJT.

osubuckeye - No offence but if you need to ask if you should lube slides with brake pad replacement this isn't for you,

Well from what I can see, it looks like you are having some problems with one of these two things. Stuck Piston or the pads were just no good. I had the same problem with my sunfire and it turned out that my brakepad had just completely fallen off. Only thing I did was simply replaced the rotor and the pads on both sides and 8k miles later, both sides still look good. If you have to replace the rotor on one side, replace both of them. Don't short change yourself on that cause its the brakes your talking about. If they were difficult to pull off then you might have a bad line or a stuck piston. It's hard to determine without taking it to a professional. I doubt that rotor is any good though.
